Surgical revascularization of critical ischemia of lower limbs: current guidelines and personal experience

چکیده
Background Peripheral arterial occlusive disease (PAOD) of lower extremities has been attributed primarily to atherosclerotic changes and the risk factor associated with atherosclerotic disease, including smoking, male gender, hypercholesterolemia, diabetes and age. The prevalence of CLI in general population as an adjusted rate of 3-5%. Critical limb ischemia is the most debilitating symptom of PAOD. The rational treatment of the CLI is surgery or endovascular approach. The present study was performed to determine the outcome of infrainguinal by-pass a popliteo-to-distal by-pass surgery for limb salvage in the treatment of the critical limb ischemia. Result of lower extremity by-pass surgery have traditionally been assessed by patency rate , uncomplicated wound healing, limb salvage, and patient survival rate.

Surgical infrainguinal revascularization for peripheral arterial disease: factors affecting patency rate
Background: Peripheral arterial disease is a source of morbidity and mortality. Surgical vascular reconstruction is a treatment option but probability of failure and complications are important concerns. In this study, we evaluated outcome of surgical infrainguinal reconstruction and factors affecting graft patency for a period of one year. Methods: In this cohort study, 85 consecutive patie...
متن کاملChronic critical limb ischemia.
BACKGROUND Some 40 000 lower limb amputations are performed in Germany each year, 70% of them in diabetics. About 80% of all major amputations may be preventable with the use of new interventional and vascular surgical procedures, particularly on the arteries of the leg and foot. We present the current state of the art in revascularization techniques and evaluate their usefulness for preservati...
متن کاملA Review of the Pathophysiology and Potential Biomarkers for Peripheral Artery Disease
Peripheral artery disease (PAD) is due to the blockage of the arteries supplying blood to the lower limbs usually secondary to atherosclerosis. The most severe clinical manifestation of PAD is critical limb ischemia (CLI), which is associated with a risk of limb loss and mortality due to cardiovascular events. Currently CLI is mainly treated by surgical or endovascular revascularization, with f...

BACKGROUND Revascularization alternatives for patients with critical limb ischemia and without adequate autogenous vein remain challenging. We reviewed our experience with the use of arterial homograft as a conduit for limb salvage in patients with limb ischemia and active lower extremity infections. METHODS A retrospective review of patients who underwent open arterial revascularization of t...

The angiosome concept provides practical information regarding the vascular anatomy of reconstructive and vascular surgery for the treatment of peripheral arterial occlusive disease and, particularly, critical lower limb ischemia. The aim of the study was to confirm the efficacy of direct revascularization with the angiosome concept (DR) for lower limb ischemia.Complementary manual searches were...
